Jack was born April 24, 1950, in Wichita, KS, the son of Fred Jack and Annabelle (Dushane) Smith.
Jack attended public school in Junction City, KS
Jack is survived by four brothers, Ronald Smith, Kenneth Smith, Leslie Smith and Joseph Smith; one sister, Joyce Anderson.
He was preceded in death by his parents.
